Discharge Planner

MetroRehab Hospital is a 37 bed hospital specialising in multidisciplinary medical rehabilitation for both inpatients and day patients. It is centrally located in Petersham, close to public transport and with ample street parking. Our specialty programs include Stroke & Neurological, Orthopaedic and Reconditioning.

We are seeking a part-time Discharge Planner to join our multidisciplinary team 21 hours/week. The role will include case management and discharge planning of inpatients at MetroRehab Hospital, including coordinating onward referrals. The Discharge Planner will work closely with the Social Worker and the Allied Health team.

To be successful in this role, you must have the following: 

• A degree in Nursing, Social Work or Allied Health
• Previous experience working within a multidisciplinary team in a hospital setting
• Demonstrated knowledge of My Aged Care, ACAT, community and residential services
• Excellent communication skills and a can do attitude
