On offer is a brilliant opportunity not to be missed. This highly rewarding role is integral to the Palliative Care team at Country Health Connect Murray Bridge. It provides Grief and Bereavement services to Palliative Care clients and their carers, family and friends at home, across their care continuum until death and through the bereavement period . Within a flexible environment that supports work life balance, you will work as a Bereavement Support Coordinator providing counselling and support to the terminally ill, their carers and families. Using a Primary health care approach, you will be responsible for the coordination, development and implementation of a range of bereavement and support services, including case management,  group work, health promotion and community development programs, which address the identified needs of the community. A strong relationship builder, you will liaise with other health professionals and stakeholders to ensure the holistic management of these clients.

Instrumental to your success in this role is substantial experience in Palliative Care, Oncology or Bereavement counselling demonstrating well rounded skills in counselling. With a successful track record dealing with community type clients, you will bring the ability to communicate effectively with clients, their families, and friends of various ages, cultures and backgrounds. Displaying compassion and empathy, you will be able to effectively manage issues of death and dying with experience dealing with clients, their carers and families in the terminal phase of life. In addition, you will have experience coordinating groups, along with a sound understanding in the management of clients in a variety of settings within the community.
