Mercy Health is looking for an Assessment Coordinator. The Assessment Coordinator is responsible for the coordination of programs including the Commonwealth Home Support Programme (CHSP) NDIS, private clients and other clients as requested, within dedicated regions to ensure that intake and assessment processes meet departmental standards in accordance with MHHC processes and CHSP Guidelines.

Reporting to the Regional Manager, the role will also be responsible for conducting client assessments, accessing the appropriate CHSP services, service development and rostering set up of clients. In addition, the role will also support growth of the program, building on the current CHSP program along with growth in private client case loads.

The Assessment Coordinator will review procedures, administration, reporting requirements and community with program stakeholders under the direction of Regional Manager.
