What You Will Be Doing

The Transition Care Program (TCP) is seeking an experienced, innovative and flexible Administrative/Ward Clerk support assistant to work as a member of the TCP team on a permanent part-time basis (up to 40 hours per f/n).

The program is a jointly funded program (DHS/Commonwealth) and it aims to assist frail aged patients to reach their maximum functioning capacity and to help determine their long term care arrangements.

What You Need

The successful applicant will need to be flexible in terms of working days and be open to the different roles within the TCP service including covering sites in both Frankston and Safety Beach. The successful candidate will need to demonstrate sound computer skills, and have an understanding of the issues affecting patient movement and flow throughout the acute and sub-acute sectors.

The successful applicant will need to be highly motivated, and enthusiastic and to be able to work both independently and as part of a team.
