About Department
This position is part of the Health Independence Program (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning.
SVHM HIP consists of integrated hub sites across several locations including in Fitzroy, Kew, North Richmond and Fairfield.
This position will be part of the HIP Chronic Disease team based in the first instance at the Fitzroy site.
This position may be required to work at any of the HIP hub sites or programs. This will be at the discretion of the HIP manager and will take into consideration appropriate skills and training.
The key aim of Chronic Disease service is to improve health outcomes to people who either frequently present, or who are at risk of presenting to hospital, because of complex medical and or social issues. CCS currently consists of multidisciplinary teams based at SVHM Fitzroy and at NRCH Hub. Clinical service delivery is undertaken in the clients’ own home and their local community.
HIP at SVHM is delivered in partnership with North Richmond Community Health (NRCH).
About the Role
An opportunity currently exists for an Allied Health Assistant (AHA) to join the HIP Complex Care Services team on a temporary, part time basis – 0.42FTE.
The Allied Health Assistant (AHA) will be responsible for clinical support and administrative assistance to our Complex Care Services. Working under supervision, the AHA will provide client intervention as directed by the qualified staff, in the clients own home environment, within the hospital setting and/or in the community. The AHA will predominantly work with Physiotherapy and Occupational Therapy to provide delegated individual client sessions and assist with joint client sessions. Future scope of the role may include working with other allied health professionals and care coordinators to support the delivery of the client care plans. The majority of the clinical role will consist of face to face contact with clients and their carers; whilst some intervention will be delivered via the telephone/telehealth.
Classification for this position will be TC2 ($60,700 per annum)
Duties
- The AHA will work under direction and supervision of allied health professionals to deliver high quality care for clients with chronic diseases and complex care needs.
- The role involves direct and indirect clinical care , maintaining an active caseload and supporting clients to develop self-management skills in relation to their health and wellbeing.
- Under direction of clinician undertake sourcing of client aids and equipment . Provision of aids and equipment and review of client satisfaction with aids and equipment.
- Clients will be seen in and within their own home/community and/or at any of the SVHM HIP locations.
- The AHA will play an important role in the non-clinical aspects to client care and service delivery . This includes administrative tasks and IT systems to ensure the daily operations of Complex Care Services.
- The AHA will facilitate, when required, some intake processes across Complex Care Services
Qualifications, Skills and Experience
- Certificate IV in Allied Health Assistance (or equivalent).
- Experience working with patients or clients that have chronic diseases and complex health and wellbeing needs
- Willingness to undertake a wide range of daily tasks (including direct client contact, administrative duties, phone calls and computer tasks)
- Willingness to provide services in a variety of locations including in the patients’ homes, local communities and within the hospital environment.
